What is able care connect home health?

Able Palms Home & Health Care Services, Inc. d/b/a Able Care Connect Home Health is a Medicare certified home health agency licensed by the State of Florida to provide both skilled nursing and private duty services to seniors in their homes throughout Pinellas County.

Depending on your unique health care needs, Able Care Connect Home Health offers a large scope of services ranging from minimal assistance to skilled clinical professionals. Our care is defined by clinical expertise, the compassion we deliver, and Platinum Service®, our service philosophy. Care plans are created in cooperation with the client, the client’s doctor, family members of the client, and the agency.

Home Health Care

Services begin with an order from your physician for home health care. Then, an appointment is set to establish a personalized Plan of Care.

Private Duty Home Care

Private duty home care agencies provide a broad range of services from medical services (not covered by insurance) to nonmedical personal care services. Our goal is to provide the necessary services to assist the individual and their families, in order for their loved one to live as independently as possible where they most want to be, in their own home.

Medicare coverage criteria for home health care

Medicare pays 100% for many home health services if all of the following criteria are met:

  1. Physician must order home health services as medically necessary and sign Plan of Care
  2. Patients must be eligible for Medicare Part A skilled services
  3. Services are intermittent, reasonable, skilled and medically necessary
  4. Patient must be homebound as defined by Medicare
